Please complete a consult, along with any pertinent labs/radiology results, and fax to 617.638.7449. Please also include the patient’s demographic information (name, address, telephone number, gender, date of birth, social security number and insurance).

Bone Density Test Requests
Please complete our bone density test requisition. For BMC physicians, this can be found in Logician. For those outside BMC, please call us at 617.638.7470 and we will be happy to fax a requisition to you.
Urgent Requests
We are an extremely busy and high volume clinic with over 26,000 annual visits. If your request is urgent, please clearly document this on your consult or call us directly.
