Assessment Center
Through our Geriatrics Assessment Center (GAC) we offer older adults a comprehensive, interdisciplinary team evaluation and help you, your family, your primary care physician, and involved agencies to develop a treatment plan that addresses medical, psychological, social and functional issues. Our location in the Yawkey Ambulatory Care Center at Boston Medical Center makes it easy for us to coordinate any specialty care you might need.
The Geriatrics Assessment Center provides a comprehensive consultation and detailed written report and recommendations for you, your family and your primary care provider.
Common issues addressed by the Geriatrics Assessment Center team include:
- Dementia and memory loss
- Chronic pain
- Depression
- Chronic heart or lung disease
- Diabetes
- Frailty
- Multiple medications
