Non-BMC Physicians
Please complete a consult and fax it, along with any pertinent labs/radiology results, to 617-638-7449. Please also include the patient’s demographic information (name, address, telephone number, gender, date of birth, social security number and insurance).

*Please include an insurance referral, if needed.

Bone Density Test Requests
Please complete our bone density test requisition. For BMC physicians, this can be found in Logician. For those outside BMC, please download the form located below or call us at 617-638-7470 and we will be happy to fax a requisition to you.
