Faculty Practice for Boston University Oral & Maxillofacial Surgery

The mission of the Boston University Oral and Maxillofacial Surgery Faculty Practice is to provide state of the art oral and maxillofacial surgical care to all our patients, encompassing the full-scope of the specialty. Most clinicians in our practice have international recognition for their expertise and have super-specialized fellowship training in their areas of interest. Our faculty members are also heavily involved in conducting innovative clinical and laboratory research and are frequently invited to give scientific presentations at national and international meetings.

Faculty Practice Outpatient Services

We offer appointments Monday-Friday from 8:00 AM – 4:30 PM.

After Hours and Emergencies

24/7 on-call coverage is available on-site. Existing patients should call 617.638.8000 and ask for the oral and maxillofacial surgery on-call resident to be paged.

Referral Form for Faculty Practice Clinic (PDF)

Most health insurance plans have some managed care component. Managed care plans require a referral from a primary care physician to cover your visit and evaluation. It is the patient’s responsibility to obtain this primary care referral. Please contact your primary care office and ask them to fax the referral to 617.638.4365. If this is a requirement of your health care plan, and an appropriate referral is not obtained, most managed care plans will deny the payments for the visit and instead, shift the cost to you.

Referral form from dentist or physician for specific procedure or reason for consultation
Bring medical records, x-rays, dental models, if applicable or given by your doctor
Contact your primary care to obtain referral authorization and fax to 617.638.4365
Bring a copy of your medical and dental insurance cards, since some dental procedures could be covered under your medical plan and we can often coordinate your benefits to get you the best coverage

Parking

Parking is available at 710 Albany Street Parking Garage. Valet parking is available in front of the Menino Pavilion (closest to Yawkey 5 practice location) at 840 Harrison Avenue.
