Sickle Cell Transition Guidebook for Medical Professionals

When young adults with sickle cell disease have to transfer their medical and non-medical care to adult services, they may find it difficult to understand all that is expected of them. This guidebook provides the necessary information and resources to help young adults transition into the adult care setting successfully. It will also help prepare these individuals for a new role as an independent adult by exploring available medical, clinical, nutritional, and social services.
