John P. Forman, MD, MSc, is a board-certified primary care physician and nephrologist at Boston Medical Center (BMC) and an associate professor at Boston University Chobanian & Avedisian School of Medicine. After receiving his medical degree from Yale School of Medicine, he completed an internal medicine residency at the University of Washington and a nephrology fellowship at Brigham and Women's Hospital/Massachusetts General Hospital.
