Pre-Arrival Support

In order to give you the most efficient evaluation, we highly recommend you connect with our worker's comp liaison to discuss your first visit.  

What to Bring to Your First Visit

If you are injured because of a work-related accident, please provide the following information at your first visit:

  • Your employer's address and phone number
  • The worker's compensation insurance carrier and adjuster name, address and phone number  
  • The claim number  
  • Date of injury  
  • All documents from prior care, including medical notes and imaging (MRIs, X-rays, CT scans)

For all worker's compensation or motor vehicle accident cases, you must also provide us with your health insurance card and a referral (if applicable). If your claim is later denied, we will then bill your health insurance plan.
